WHAT TO EXPECT
WEIGHT MONITORING PROTOCOL
Scales placed in the home at admission.
Weights recorded daily and reviewed at each visit.
Alerts Triggered for:
2lbs ↑ in 24 hr
5lbs ↑ in 1 Week
Progressing edema or abdominal distention.
If Unable to weigh, an abdominal girth measurment will be taken. This will be on the POC.
Alert for increase greater than 3cm
ACIVE NURSING
Call PCP from in the home (or if notified on phone visit) for the following:
Incresed Edema
Weight Gain
Decreased Output
Chest Pain
Shortness of Breath
Obtain an intervention, whether it be labs, med change, fluid restriction, etc.
ADMISSIONS: SOC/ROC
In-home visits within 48 hours of admission; if visit is on Friday, a followup call by RN on call will be made and an in -hom visit made on Monday.Â
Front-load visits for the 1st 7 days, minimum 2 visits in first 7days
Phone visits performed each day for the first 5 days.Â
